Application Fees: An application fee of Rs.150 (for posts with GP between Rs.1800 and Rs.2400) and Rs.250 (for posts with GP from Rs.2401 and above) has to be deposited in any Branch of the Punjab National Bank in a specially opened account a/c no 3247002100149692. For remittance of fee, candidates have to generate the prescribed challan from ESIC website.
Candidates should collect a copy of the challan duly signed and stamped along with deposit scroll / journal number. This copy of challan has to be sent to the ESIC along with the hard copy of the application form.
Female / SC / ST / Ex. SM / PWD / Govt. Employee candidates are exempted from payment of fees.
Fee once paid will neither be refunded nor adjusted under any circumstances.
Selection Procedure: Final selection would be done on the basis of candidates’ performance in the Written Test and Interview for the posts of Dietician, O.T. Technician, ECG Technician and Social Worker only Written Test for remaining posts.
Corporation reserves the right to conduct the written or online examination.
There shall be negative marking for wrong answer. For each wrong answer 0.33 mark will be deducted.
TA shall be paid to SC/ST candidates for interview only as per Govt. of India orders.

How to Apply: Interested eligible candidates should apply online through ESIC website http://www.esicmhchd.org till 26 December 2012. On successful submission of online application, candidates should take its print out and send it along with copies of relevant documents / certificates and Commisssion’s copy of the fee deposit challan to “ESIC ‐ Model Hospital, Industrial Area Phase‐II, Ramdarbar, Chandigarh 160002” through Registered Post / Speed Post so as to reach the said office by the prescribed last date.
On the envelope containing the application, candidates should be super-scribe “Application for the post of ______, Post Code No. ______ State __________.”
Candidate should furnish correct Mobile Number / e-mail address to receive the registration confirmation. They will receive registration confirmation by SMS / e-mail after submitting the online application. They should have valid
e-mail ID, this will help him/her in getting Admit Card /Interview advices, etc. by e-mail. No hard copy would be sent by post
NOTE: The vacancies are in various ESIC Hospitals of ESI Corporation of the specified State. Those willing to accept posting anywhere in the Specified State and are willing to be transferred in any part of India on promotional posts need apply.
Candidates are advised to submit only one application because written examination for all the categories of posts will be held on the same day and same time.
On successful submission of online application, a registration number and password will be generated. Candidates should note it down for future reference. Candidates can reopen the saved data by using this Registration number & password and can edit the particulars, if needed. Once the application is filled completely, candidate should submit the data. No change / edit will be allowed after the last date of submission of data. The registration at this stage is provisional. Candidates should keep a copy of the Application printout and Fee Challan Receipt for their record.
