- Candidates must be registered with the UP Nurses & Midwives Council, but candidates registered with other state nursing councils as nurses and midwives are also eligible to apply, provided they submit a registration certificate of UP Nurses & Midwives Council within 90 days of their provisional joining at district if shortlisted.

UPNHM CHO Eligibility Criteria 2024: Reservation

- Reservations will be given as per the applicable reservation rule to candidates who are original inhabitants of Uttar Pradesh (domicile of Uttar Pradesh only).
- Candidates who do not have a domicile in Uttar Pradesh will be considered in the unreserved category and may apply only in the unreserved category.
- Candidates belonging to the SC, ST, OBC (NCL), PWD and EWS categories can avail of reservation benefits by submitting a valid SC, ST, OBC (NCL), PWD and EWS certificate while filling out the online application form.
- In the case of female candidates, caste certificates issued from the father’s side only will be treated as valid.
- Candidates claiming the benefit of reservation/age relaxation must obtain the category certificate issued by the competent authority
UPNHM CHO Eligibility Criteria 2024: Documents Required
Candidates are required to upload the following documents while filling out the online application form to support their UPNHM CHO eligibility claims and avail themselves of the age relaxation/reservation benefits prescribed by the officials. The list of required documents is as follows.
- Mark sheet of high school.
- Mark sheet of Intermediate.
- All mark sheets of BSc (Nursing)/PB BSc Nursing.
- Certificate from the Principal of the concerned Institution regarding Integration of Middle-Level Health Providers (MLHP)/CCHN Course in Basic B.Sc. (Nursing) and PB B.Sc. Nursing.
- QR Based COVID experience certificate issued by the concerned district. QR-based COVID experience certificate issued on prescribed proforma as per the PE specification only shall be considered valid.
- Valid category certificate.
- Valid photo ID and address proof.
- UP Nurses & Midwives Council Registration Certificate/Other State Nursing Council Registration Certificate.
- Domicile certificate.
- PwD, Dependent of Freedom Fighters and ex-servicemen Certificate.
